Under a physician’s direction and supervision, the following services can be provided in the patient’s home on an intermittent basis.

Skilled care, delivered in the home
Each discipline below is provided on an intermittent basis under your physician’s direction and supervision.
Skilled Nursing
Licensed nurses deliver clinical care and education in the home, coordinating with your physician at every step.
- Treatment and injections
- Dressing change and wound care
- Catheter care
- Supervision of medications & refills
- Restorative care
- Education on use of home care equipment
- Respiratory rehabilitation
- Health education program
- Diabetic care and teaching
- Bowel and bladder training
- Care of the terminally ill
- Blood sugar testing
- Home medical equipment
- I.V. fluids and antibiotic therapy
- Pre-op and post-op care
- Ostomy care and teaching
- Parenteral and enteral feeding
- Tracheostomy care
- Decubitus (pressure) ulcer care
- Pain management
Physical Therapy
- Restoration of previous level of function, range of motion, and strength
- Rehabilitation and education on the use of adaptive equipment
- Education of patient, family, and caregiver on home safety, ambulation, and transfers
- Establish a Home Exercise Program
Occupational Therapy
- Therapeutic exercises to promote independence in the activities of daily living (ADLs)
Speech Therapy
- Assist in the restoration of speech and language
- Rebuild the ability to communicate
Home Health Aide
- Assist patient with personal care
- Assist with safe ambulation and transfer
- Meal preparation and light housekeeping
Medical Equipment & Supplies
- Arrangement for these services will be made to meet the patient’s needs
Please note: All services are provided on an intermittent basis under the direction and supervision of the patient’s physician. Eligibility for specific services is determined by the physician and the home health agency staff following an initial assessment.
