Home care is the provision of health care in the patient’s home. Patients feel more confident and secure because of being in his or her usual place of residence, where family and friends can participate in providing care. A team of healthcare professionals works together with the patient and his or her family, friends, or caregiver to deliver care to the patient under the supervision of a licensed physician.

Home care includes skilled nursing, therapy, home health aide, and social services provided to the patient on a one-on-one basis. Different levels of treatment, training, and education are customized to a patient’s needs and may involve family, friends, and caregivers as necessary. Patients discharged home from a healthcare facility are assured of continuity of skilled care.

Anyone can qualify for home care services as long as the criteria for admission to home care services are met. This will be determined by your physician and the skilled agency staff, who will perform an initial assessment.

Anyone may request home care services — the physician, the patient, family, and friends.

Approved services are covered 100% by Medicare and Medi-Cal. Private insurance coverage varies with individual policies. Self-pay or private pay is handled on a case-by-case basis. We process insurance verification and eligibility, and submit claims on the patient’s behalf.

Under a physician’s direction and supervision, services can be provided in the patient’s home on an intermittent basis, including skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, home health aide, and medical equipment and supplies. Our office hours are Monday–Friday, 9:00 AM – 5:30 PM. An on-call nurse is available after office hours, on weekends, and on holidays.
