This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

This notice is provided as required by the Health Insurance Portability and Accountability Act (HIPAA) and applicable California law. It is a template intended to describe our privacy commitments. Pioneer Home Health Services Inc. should confirm the final content with its privacy officer and legal counsel before distribution.
Our pledge regarding your health information
Pioneer Home Health Services Inc. is committed to protecting the privacy of your protected health information (PHI). We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
How we may use and disclose your health information
We may use and disclose your health information, without your separate authorization, for the following purposes:
- Treatment. To provide, coordinate, and manage your home health care — for example, sharing information among the nurses, therapists, aides, social workers, and physicians involved in your plan of care.
- Payment. To obtain payment for services, including insurance verification, eligibility determination, billing, and claims submission to Medicare, Medi-Cal, or your insurer.
- Health care operations. For quality assessment, staff training and competency review, accreditation, licensing, and care coordination.
- As required by law. When federal, state, or local law requires the use or disclosure.
- Public health and safety. To prevent a serious threat to health or safety, report communicable diseases, or report suspected abuse or neglect, as permitted or required by law.
- Health oversight, legal, and regulatory. To health oversight agencies, in response to court orders or subpoenas, and to coroners, medical examiners, or funeral directors as permitted by law.
Uses and disclosures that require your written authorization
Other uses and disclosures — including most uses of psychotherapy notes, uses for marketing, and any sale of PHI — will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
Your rights regarding your health information
- Right to inspect and copy your health and billing records, subject to limited exceptions.
- Right to request an amendment of information you believe is incorrect or incomplete.
- Right to an accounting of certain disclosures we have made.
- Right to request restrictions on certain uses and disclosures of your information.
- Right to request confidential communications by alternative means or at an alternative location.
- Right to a paper copy of this notice upon request, even if you have agreed to receive it electronically.
- Right to be notified following a breach of unsecured protected health information.
Our responsibilities
We are required to maintain the privacy of your PHI, to provide you this notice, to abide by its terms, and to notify you if we are unable to agree to a requested restriction. We reserve the right to change this notice and to make the revised notice effective for information we already have as well as for information we receive in the future. The current notice will be available at our office and on this website.
