Patient Rights & Responsibilities

Every patient who receives care from Pioneer Home Health Services is entitled to the following rights and shares in the following responsibilities.

Your rights

As a patient, you have the right to…

  • Be treated with consideration, respect, and full recognition of your dignity and individuality.
  • Receive care without discrimination on the basis of race, color, national origin, disability, age, religion, sex, or source of payment.
  • Be informed, in advance, about the care to be furnished and any changes to your plan of care.
  • Participate in the planning of your care and in decisions about your treatment.
  • Have your property and person treated with respect in your home.
  • Confidentiality of your clinical records and protected health information.
  • Be informed of the charges for services and your responsibility for payment.
  • Be informed of your right to refuse treatment, and to be told the expected consequences of doing so.
  • Voice grievances or complaints about your care without fear of discrimination or reprisal.
  • Be informed about how to contact the agency and the state home health hotline regarding concerns.
  • Receive a timely response to your requests for service.
  • Be advised of the agency’s on-call availability after office hours, on weekends, and on holidays.
Your responsibilities

As a patient, you are asked to…

  • Provide accurate and complete health information to your care team.
  • Participate in your plan of care and follow the agreed-upon instructions.
  • Tell your care team about changes in your condition or concerns about your care.
  • Provide a safe environment for care to be delivered in your home.
  • Notify the agency in advance if you will be unavailable for a scheduled visit.
  • Meet your financial responsibilities for services as agreed.
  • Treat agency staff with courtesy and respect.
